Thank you for the opportunity to work with you and your child. All of us here at Theraplay are focused on helping your child develop to their fullest potential and will do our very best to facilitate their learning and growth. We have developed the following guidelines to help welcome you to our center and make your therapy experience as enjoyable and easy as possible.
Please arrive at least 15 minutes early for your first visit to complete the paperwork process.
Please print and complete the New Patient Information Packet. Bring completed packet with you to your child's evaluation.
The Family Guide to Theraplay is general information about our office for you to read and keep at home. The HIPAA Privacy Notice is for your review and files.
We ask that families and caregivers respect our staff members’ privacy when using social media. At Theraplay, we strive to maintain your child’s and family’s confidentiality at all times, including on social media. We encourage you to “like” Theraplay’s Facebook page or follow us on Instagram and Twitter to keep up to date on Theraplay news and events rather than connecting with staff through their personal social media pages. Our staff are prohibited from “friending” any current patients and their families.
All therapeutic services provided by Theraplay require a physician’s prescription. After consulting with your physician, your family can contact the Theraplay outpatient office that is convenient for you. The front desk staff at Theraplay will verify your child’s insurance coverage for therapy services, and discuss all coverage and financial responsibilities with your family. An initial evaluation will then be scheduled. Once the evaluation is completed, a copy of the evaluation will be sent to the physician, along with a completed Letter of Medical Necessity for the physician’s signature.
Theraplay provides patients with the best, yet affordable, therapeutic services possible. Theraplay is an approved provider for most insurances, including HMO’s, PPO’s and commercial carriers. Theraplay’s reimbursement staff will assist your child’s doctor’s office in obtaining any necessary precertifications and/or authorizations needed prior to the initiation of therapeutic services.
